“Good Faith Estimate”
Welcome to our practice! We believe that informed clients are empowered clients. It’s essential for you to understand the benefits of your insurance policy. The No Surprises Act is a crucial law that enhances transparency and protects you from unexpected medical bills. We encourage you to explore and make the most of your insurance policy’s advantages, and here’s how it relates to your care with us:
1. Protection from Surprise Bills: The No Surprises Act ensures that you’re shielded from unexpected and potentially high medical bills, especially if you receive care from providers who are not in your insurance network without your consent. This protection ensures that you won’t be financially responsible for out-of-network charges in emergency situations or when you receive care at an in-network facility without your informed consent.
2. Advance Explanation of Benefits (AEOB): Before you receive care, your insurance provider is obligated to provide you with an Advance Explanation of Benefits (AEOB). The (AEOB) is a critical document provided by health insurance companies to policyholders. It offers a detailed overview of the expected costs and coverage related to a specific medical service or procedure before the service is performed. The AEOB serves as a tool to help policyholders understand their financial responsibilities, potential out-of-pocket expenses, and the extent of coverage provided by their insurance plan. This document is a valuable resource that outlines your cost-sharing responsibilities and the network status of your providers. It’s your tool for understanding and taking control of your healthcare expenses.
3. Billing Transparency: We’re committed to providing you with clear and transparent billing information. After setting up a “patient portal” account, you will be able to view information about services rendered and make payments.
4. Independent Dispute Resolution (IDR): If, in rare cases, there’s a dispute about the cost of out-of-network care, the (IDR) process comes into play. This process is designed to protect you from excessive charges and ensures fairness.
We encourage you to take an active role in your healthcare journey. Review your insurance policy, ask questions, and become familiar with your rights and protections under the No Surprises Act, your understanding of your policy’s benefits will contribute to a smoother and more predictable healthcare experience. Feel free to reach out if you have any questions or need guidance. We’re here to assist you as best we can in making informed decisions and managing your healthcare expenses effectively.
Attendance Policy
At Open Water Wellness, PLLC, your care and well-being are our greatest concern. We have an attendance policy for care at our practice. First, and most importantly, regular attendance at scheduled follow-up appointments are crucial to the success of your treatment. Additionally, missed appointments complicate access to services for others and create a barrier for continuity of care.
Being respectful of your time, other clients, and our provider’s time, we require a minimum of 24-hour notice prior to any canceled appointment. This will allow us time to fill the appointment slot to maximize access to others seeking services. Failure to provide 24-hour notice prior to missing a scheduled appointment or no-showing for an appointment will be treated the same.
• Each missed appointment will incur a $50.00 charge.
• lf more than 4 (four) appointments are missed within a 6-month period with less than 24-
hour notice or without notice, the client’s care can be terminated with our practice.
We will make every effort to work with you to schedule appointment times/days that are conducive to your schedule and our availability. However, no exceptions will be granted for this policy.
We appreciate your cooperation and understanding.
